Emergency Form

• • To register for Church School you must complete one Emergency Form for each child (4 children = 4 separate forms).

  • Complete the on-line Emergency Form below and click on the "Submit" button at the bottom.

  • That is all. There is no need to print this Emergency Form on paper and hand it in to the Church School.

  • You must also complete one Registration Form for the entire family(accommodates up to 4 children).
